You can configure the guestbook to automatically email you when someone attempts to post a comment. You will then have the opportunity to screen the comment before allowing it to appear on your site. The benefits of that feature are obvious.

Also, even though it's remotely hosted, you truly have total control over the appearance of your guestbook, allowing you to blend it with the look of your site.

To me, the only drawback is the difficulty in getting it to validate as Strict XHTML 1.0. But this would not be an issue for most webmasters.

Also, I know you don't want a gueatbook with ads but the small banner that's required is actually little more than a discreet text link that must appear in the top portion of the page. However, if you position it wisely, it's not distracting to your users whatsoever.
