Are you wanting a form for people to order your pre made sites ?

OR

Are you wanting a contact form for people to email you from your sites ?

Just reply and I will help best I can but please see the link above. A form is pretty much customizable to what you want. You could have a field in with a drop down menu and a field for their Paypal address etc.
